These changes will remain in effect until otherwise directed by University of Michigan leadership and public health officials. These forms have been developed to assist you with administering your program.  Each form has been developed in consultation with program/event administrators and the Office of the General Counsel to ensure they are helpful with addressing the health, safety and security of children and meet U-M requirements.

If photos or videos are taken during participation in the program
Suggested for participants in U-M programs. Consider the following:

  • Program duration
  • Age of children
  • Nature of activities
  • Health of participant children
Required if children need to take medication during the program
Required if program staff wish to make over-the-counter medications available
Required if children under the age of 12 are participating in the program
Required if program chooses to allow children to self-medicate
